  • Do providers accept insurance?

    Most of our providers accept insurance. When calling to schedule an appointment, please make the office aware of the type of insurance you have so that we can match you with a provider that meets your needs and accepts your insurance.

  • Are you accepting new patients?

    Yes, please contact the office for in person and or virtual availability.

  • How do I know which provider to choose?

    Our clinical director will work with you to determine which provider would be the best fit for you. Call our clinical director at (248) 451-9085 or email Admin@dmbassociates.com/

  • What do I need for my first appointment?

    Our New Patient Paperwork can be found on our website or provided to you at the office during your first in person appointment. In addition to the paperwork, copies of your insurance card and driver license will also be made by our office staff.

  • How do I pay my bill?

    Please call our office at 248-451-9085 to call and make a payment over the phone by pressing extension 10, mail a check to 1750 S. Telegraph Road, Bloomfield Hills, Suite 101, or fill our a credit card authorization form. Credit cards are typically billed on Fridays.

  • I have a question about billing.

    Please email our biller at OfficeManager@DMBandAssociates.com or leave a message on extension 18.

  • Do providers have in-person or virtual appointments?

    Our providers offer both in person and virtual appointment options. Please speak to your provider for details regarding scheduling.
